Medicare GLP-1 Bridge: Who Qualifies for $50-a-Month Wegovy, Zepbound or Foundayo
Who qualifies for Medicare GLP-1 Bridge, which drugs it covers for $50 a month, how to enroll, and what to do if you don't meet the BMI or plan rules.
Medicare GLP-1 Bridge lets people with Part D coverage get Wegovy (pen or pill), Foundayo or the Zepbound KwikPen for $50 a month from July 1, 2026 through December 31, 20271,2. You must be 18 or older, meet a BMI threshold (35, or 30 or 27 with certain conditions), and not already get a GLP-1 through your plan.
The Bridge is a temporary program that runs outside your regular Part D coverage, with its own eligibility rules, its own approval step and its own claims processor2. This guide walks through each rule, the drugs on the list, the enrollment steps and the cash options if you don't qualify.
Who qualifies for Medicare GLP-1 Bridge?
You must meet all four of these requirements, which come straight from Medicare's fact sheet1:
| Requirement | What it means |
|---|---|
| 1. You have Medicare Part D drug coverage | Either a standalone Medicare drug plan or a Medicare health plan that includes drug coverage. Some special plan types don't count: private fee-for-service plans, cost contract plans and PACE organizations. |
| 2. Your Medicare drug plan doesn't already cover a GLP-1 for you | If your plan has been paying for a GLP-1 for any reason, you keep getting it through your plan. GLP-1s here means semaglutide, tirzepatide, orforglipron, dulaglutide and liraglutide. |
| 3. You don't have type 2 diabetes, moderate-to-severe sleep apnea or fatty liver disease | People with these conditions are told to contact their drug plan, which may already cover a GLP-1 for them. |
| 4. You're at least 18 and meet one of the BMI rules below | Your doctor calculates BMI from your height and weight. |
The BMI rule has three levels. Meeting any one of them is enough1:
| Your BMI | Plus one of these |
|---|---|
| 35 or higher | Nothing else needed |
| 30 or higher | Certain types of heart failure, high blood pressure that's hard to control, or chronic kidney disease (stage 3a or above) |
| 27 or higher | Prediabetes, or a past heart attack, stroke, or blocked arteries in your legs or arms |
Lilly's Bridge page adds a useful detail for people already on treatment: the BMI criteria can be met as of when you started a GLP-1, not only today4. If you've already lost weight on a cash-pay GLP-1, ask your doctor to document your starting BMI. Our GLP-1 eligibility checker gives a quick BMI-based read; confirm the Bridge rules themselves with your doctor.
Which drugs are covered, and which are not?
The list is short, and the Zepbound restriction catches people out1,4:
| Drug | Covered by the Bridge? | Note |
|---|---|---|
| Wegovy injection (pen) | Yes | $50 a month |
| Wegovy tablet (pill) | Yes | $50 a month |
| Foundayo (orforglipron) tablet | Yes | $50 a month, 30-day supply |
| Zepbound KwikPen | Yes | $50 a month; pen needles are bought separately |
| Zepbound vial | No | Self-pay only |
| Zepbound single-dose pen | No | Not covered |
| Ozempic, Mounjaro and other diabetes GLP-1s | No | Not on the Bridge list |
If you and your doctor choose Zepbound, the prescription has to be written specifically for the KwikPen4. A vial prescription will not go through the program.
Why are people with diabetes, sleep apnea or fatty liver excluded?
Because their regular Part D plan may already cover a GLP-1 for those conditions. Medicare's fact sheet says so directly: if you have type 2 diabetes, moderate-to-severe sleep apnea or fatty liver disease, contact your Medicare drug plan, since it may already cover a GLP-1 for you1. The Bridge is aimed at people whose reason for a GLP-1 is weight management alone.
The same logic applies to prescriptions. Lilly notes that if Zepbound is prescribed for obstructive sleep apnea with obesity, you're not eligible for the Bridge; that use goes through your plan instead4.
How do you enroll?
There's no separate sign-up form for patients. The process runs through your doctor and your pharmacy1,4:
| Step | What happens | Who does it |
|---|---|---|
| 1 | Talk about whether a covered GLP-1 is right for you and whether you qualify | You and your doctor |
| 2 | Your doctor sends a prescription for a covered drug to a pharmacy, retail or LillyDirect | Your doctor |
| 3 | The pharmacy asks for your Medicare ID number (it can look it up from the last four digits of your Social Security number) and checks eligibility | Pharmacy |
| 4 | Once you're confirmed eligible, your doctor submits a prior authorization form to Medicare | Your doctor |
| 5 | You get a letter from Medicare saying whether your medicine is covered; this can take several days | Medicare |
| 6 | You pick up a one-month supply and pay $50 | You |
Refills don't need a new approval as long as you stay on the same drug, even if your dose goes up1. Medicare says the prior authorization is valid, including for refills and dose changes, through December 31, 20273. Lilly notes that a new approval is needed only if you switch to a different GLP-14.
Does the $50 count toward my Part D deductible?
No. The Bridge sits outside your regular drug coverage, so the Part D deductible doesn't apply to it, and the $50 doesn't count toward your deductible or your yearly out-of-pocket limit1,2. These drugs also aren't eligible for the Medicare Prescription Payment Plan1.
The price is the same for everyone: $50 a month regardless of income1. That cuts both ways. There's no Extra Help (low-income subsidy) applied to it, so a person who gets Extra Help on other drugs still pays $502.
Does Medicare Advantage count?
Yes, if your Medicare Advantage plan includes drug coverage. The fact sheet counts "a Medicare health plan that includes drug coverage" alongside standalone drug plans1. The exceptions are the special plan types listed above: private fee-for-service, cost contract plans and PACE. If you're not sure which type you have, call 1-800-MEDICARE (1-800-633-4227; TTY 1-877-486-2048)1.
What if you don't qualify?
You can still pay cash, but not through your Medicare benefit. The manufacturers' self-pay offers, including the coupons listed on TrumpRx.gov, are for self-pay patients only; you can't use them while claiming Medicare benefits, and what you pay doesn't count toward Part D costs8. Here are the main cash routes in October 2026:
| Cash route | Starting price | Maintenance price |
|---|---|---|
| Wegovy pill (Novo Nordisk self-pay) | $149/mo at 1.5 mg5 | $299/mo at 9 and 25 mg5 |
| Wegovy pen (Novo Nordisk self-pay or TrumpRx coupon) | $199/mo for the first two fills5 | $349/mo; $399 for HD 7.2 mg5 |
| Zepbound vial or KwikPen (LillyDirect) | $299 per fill at 2.5 mg6 | $399 at 5 mg; $449 at 7.5 to 15 mg if refilled within 45 days6 |
| Foundayo pill (LillyDirect) | $149/mo at 0.8 mg7 | $299/mo at 9, 14.5 and 17.2 mg (the top two doses if refilled within 45 days)7 |
| Compounded semaglutide via telehealth | Median $199/mo across 352 programs | Usually one price at every dose |
| Compounded tirzepatide via telehealth | Median $273/mo across 332 programs | Usually one price at every dose |
The compounded medians come from our price index, and most of those plans include the clinician visit. Compounded medication is not FDA-approved, so read is compounded GLP-1 legit and safe before you choose that route. For a side-by-side look at the cash routes, see the TrumpRx GLP-1 price guide.
What to do next
If you think you qualify, the next step is a conversation with your doctor. Bring your Medicare card, your BMI if you know it, and any diagnosis that counts toward the 30 or 27 BMI levels. If you're choosing between the covered drugs, our Zepbound vs. Wegovy comparison covers the head-to-head trial, and remember that only the Zepbound KwikPen is on the list. If you'd rather take a pill, see Foundayo cost and how to get it.
If you don't qualify, or your doctor won't prescribe, compare the cash routes. The no-insurance board lists telehealth programs with their month-to-month prices; the brand-name board lists the ones that prescribe Wegovy and Zepbound. Sesame Care (Partner) lets you choose a named clinician and prescribes brand-name Wegovy and Zepbound at cash-pay rates, and CoreAge Rx (Partner) sells compounded semaglutide at one flat price at every dose. The 2-minute quiz matches you on budget and medication preference, and our GLP-1 answers page covers the most common cost and access questions. Frequently asked questions
Does Medicare cover Zepbound for weight loss?
Through Medicare GLP-1 Bridge, yes, but only the Zepbound KwikPen. Eligible Part D enrollees pay $50 a month from July 1, 2026 through December 31, 2027. Zepbound vials and the single-dose Zepbound pen are not covered.
Does Medicare cover Wegovy for weight loss?
Yes, through Medicare GLP-1 Bridge. Wegovy injections and Wegovy tablets are both covered for $50 a month if you have Part D coverage, meet the BMI criteria, and don't have type 2 diabetes, moderate-to-severe sleep apnea or fatty liver disease.
What BMI do you need for Medicare GLP-1 Bridge?
A BMI of 35 or higher; or 30 or higher with certain heart failure, hard-to-control high blood pressure or chronic kidney disease (stage 3a or above); or 27 or higher with prediabetes or a past heart attack, stroke or blocked arteries in the legs or arms.
Does the $50 count toward my Part D out-of-pocket limit?
No. The Bridge runs outside your Part D coverage, so the $50 doesn't count toward your deductible or yearly out-of-pocket limit, and Extra Help doesn't lower it.
