TrumpRx GLP-1 Prices vs Telehealth: Wegovy, Ozempic, Zepbound in 2026
What TrumpRx charges for Wegovy, the Wegovy pill, Ozempic and Zepbound, which doses each price covers, and how they compare with telehealth plans.
TrumpRx.gov lists the Wegovy pill from $149 a month, Wegovy pens and Ozempic from $199, and Zepbound vials from $299 for cash-paying patients1. It is a discount, not a prescriber: you need your own prescription. Compounded semaglutide through telehealth runs a median $199 a month across 352 programs we track, and tirzepatide $273, usually with the prescriber included.
Those headline numbers are real, but three of the four are starting prices, and the price you pay in month three is often higher. This guide sets out what each TrumpRx price covers, what happens after the first fills, and which route makes sense depending on whether you already have a prescriber, want the brand-name drug, or simply need the lowest monthly cost.
What is TrumpRx, and does it give you a prescription?
TrumpRx.gov is a federal price-comparison site that lists manufacturer cash prices for brand-name drugs under what it calls "Most-Favored-Nation pricing"1. For the GLP-1s, it does not sell the medicine itself and it does not employ clinicians. Every GLP-1 page requires that you are a US resident, at least 18, and already hold a valid prescription2.
What TrumpRx does is hand you to the place that fills the order. For Wegovy pens, the Wegovy pill and Ozempic, it shows a coupon (with BIN, PCN and group numbers) that you present at a retail pharmacy that stocks the drug2,3,4. For Zepbound, it sends you to LillyDirect, because Lilly manages every Zepbound vial order through its own pharmacy service5.
So TrumpRx answers the price question. It doesn't answer the prescription question. If you don't have a doctor who will write the prescription, you still need one: your own physician, an in-person clinic, or a telehealth program.
What does each GLP-1 cost on TrumpRx?
These are the prices TrumpRx and the manufacturers' own pages showed in October 2026. All are self-pay prices.
| Drug | TrumpRx headline | What the headline covers | After that, or at higher doses | Filled through |
|---|---|---|---|---|
| Wegovy pill (semaglutide) | $149/mo | The 1.5 mg starting tablet3 | $199 at 4 mg; $299 at 9 mg and 25 mg6 | Retail pharmacy coupon |
| Wegovy pen (semaglutide) | $199/mo | First two monthly fills at 0.25 mg or 0.5 mg, new patients only2 | $349/mo at 0.25 to 2.4 mg; $399/mo for Wegovy HD 7.2 mg2 | Retail pharmacy coupon |
| Ozempic (semaglutide) | $199/mo | First two monthly fills at 0.25 mg or 0.5 mg, offer runs 11/17/2025 to 12/31/20264 | $349/mo at 0.25 to 1 mg; $499/mo at 2 mg4 | Retail pharmacy coupon |
| Zepbound vial (tirzepatide) | $299/mo | The 2.5 mg starting vial5 | $399 at 5 mg; $449 at 7.5 to 15 mg if you refill within 45 days7 | LillyDirect |
Two details change the math. Ozempic is FDA-approved for type 2 diabetes, not for weight management8, so a prescription for weight loss is off-label and many clinicians will write Wegovy instead. And Lilly defines a "month" of Zepbound as 28 days7, so a year on Zepbound is 13 fills, not 12.
Which doses do those prices cover?
This is where most readers get caught. Both injectable GLP-1s start low and step up every four weeks, and the cheapest prices sit at the starting doses.
Wegovy pen. The $199 price applies only to a new patient's first two fills, and only at the 0.25 mg and 0.5 mg starting doses. From then on, every dose from 0.25 mg up to the 2.4 mg maintenance dose is $349, and the 7.2 mg HD dose is $3992. Novo Nordisk's own price page shows the same figures6.
Wegovy pill. TrumpRx displays the 1.5 mg tablet at $149 and notes that "self-pay pricing varies by dose"3. On Novo Nordisk's page, 4 mg is $199 and the 9 mg and 25 mg tablets are $2996.
Zepbound. TrumpRx defaults to the 2.5 mg vial at $299 and lets you pick any strength from 2.5 to 15 mg5. On LillyDirect the vial is $399 at 5 mg and $449 at 7.5, 10, 12.5 and 15 mg, as long as you refill within 45 days. Miss that window and the regular price applies: $499 at 7.5 mg and $699 at 10 mg and above7. Lilly also notes that 2.5 mg is a starting dose, not a maintenance dose7. Our LillyDirect Zepbound price guide covers the vial and the KwikPen side by side.
In practice, a person who reaches the usual maintenance dose pays about $349 a month for Wegovy pens or $449 per 28-day fill for Zepbound, not the headline price.
TrumpRx vs NovoCare vs LillyDirect vs telehealth: which route fits you?
TrumpRx doesn't create a new price. It points at the manufacturers' self-pay prices, which you can also reach through NovoCare, LillyDirect or a retail pharmacy. The real choice is between those brand-name routes and a telehealth program, which bundles the prescriber with either brand-name or compounded medication.
| Your situation | Route that usually fits | Why |
|---|---|---|
| You already have a prescriber and want brand-name Wegovy or Ozempic | TrumpRx coupon or Novo Nordisk's self-pay pharmacy | Same manufacturer price; pick whichever pharmacy is convenient2,6 |
| You already have a prescriber and want brand-name Zepbound | LillyDirect (TrumpRx links there) | Lilly fills every self-pay Zepbound vial order5 |
| You want brand-name medication but have no prescriber | A telehealth program that prescribes brand-name GLP-1s | You get the visit and the prescription; the drug is billed at the manufacturer's price on top |
| You need the lowest monthly cost | A compounded telehealth plan | Prescriber included, and the median is below the brand maintenance price (see below) |
| You have Part D coverage | Check Medicare GLP-1 Bridge first | $50 a month if you qualify9 |
If you want brand-name medication through telehealth, look at the visit fee as well as the drug price. Sesame Care (Partner) lets you choose a named clinician at a posted price, then prescribes Wegovy or Zepbound with the medication billed separately at cash-pay rates, so you pay a care subscription plus the manufacturer price. Found (Partner) offers brand-name Wegovy alongside compounded plans and the Foundayo pill. Our brand-name GLP-1 board lists every program that prescribes the brand-name drugs.
Is compounded still cheaper?
Yes, in most cases. Compounded semaglutide and tirzepatide are prepared by licensed pharmacies, and the FDA notes that compounded drugs are not FDA-approved, which means the agency does not verify their safety, effectiveness or quality before they are sold10. That's the trade-off behind the lower price. Here's what the month-to-month market looks like across the telehealth programs in our price index:
| Compounded, month-to-month | Low end (10th percentile) | Median | High end (90th percentile) | Programs priced |
|---|---|---|---|---|
| Semaglutide | $125 | $199 | $299 | 352 |
| Tirzepatide | $161 | $273 | $399 | 332 |
Those prices usually include the clinician visit and shipping, which a TrumpRx or manufacturer price does not. Against brand-name maintenance pricing ($349 for Wegovy pens, $449 per fill for Zepbound at 7.5 mg and above), the typical compounded plan is cheaper. Against the brand-name starting prices, the gap is small or gone: the $149 Wegovy pill and the $199 Wegovy intro offer sit at or below the compounded semaglutide median.
What decides it is how long you stay on the drug. Most people step up to a maintenance dose within a few months, and that's when brand-name prices rise. For a full cost breakdown by molecule, see semaglutide cost without insurance and tirzepatide cost without insurance. For the quality question, see compounded vs. brand-name GLP-1s.
Can you use TrumpRx with insurance or Medicare?
No. The TrumpRx coupons are for self-pay patients only. You can't use them while claiming benefits through Medicare, Medicaid, the VA, TRICARE or a private plan, and you can't count what you pay toward an insurance deductible or out-of-pocket limit2,4. TrumpRx itself suggests checking your insurance copay first, because it may be lower5.
If you're on Medicare, there's a separate program to check first. Since July 1, 2026, Medicare GLP-1 Bridge covers Wegovy (pen or pill), Foundayo and the Zepbound KwikPen for $50 a month for people with Part D coverage who meet its BMI and health criteria9. Our guide to who qualifies for Medicare GLP-1 Bridge covers the rules step by step. If you have commercial insurance, see does insurance cover GLP-1 weight loss.
What to do next
Start with the prescription, because no price matters until you have one. If you already have a prescriber and want the brand-name drug, compare the TrumpRx coupon with the manufacturer's own pharmacy and budget for the maintenance price, not the intro price. If you need a prescriber, decide first between brand-name and compounded medication:
If you want brand-name medication, the brand-name GLP-1 board lists programs that prescribe Wegovy and Zepbound.
If the lowest monthly cost matters most, the no-insurance board and the semaglutide board show compounded plans with their month-to-month prices. CoreAge Rx (Partner) prices compounded semaglutide at one flat monthly rate at every dose.
If you're not sure which way to go, the 2-minute quiz asks about budget, medication preference and insurance, and matches you with programs that fit. If Zepbound vs. Wegovy is the open question, read Zepbound vs. Wegovy first.
Frequently asked questions
Does TrumpRx prescribe Wegovy or Zepbound?
No. TrumpRx lists manufacturer cash prices and gives you a pharmacy coupon (or, for Zepbound, sends you to LillyDirect). You need a valid prescription from your own doctor or a telehealth program before you can use it.
Is the $199 TrumpRx Wegovy price permanent?
No. The $199 price covers a new patient's first two monthly fills at the 0.25 mg or 0.5 mg starting doses. After that, Wegovy pens are $349 a month at every dose up to 2.4 mg, and Wegovy HD 7.2 mg is $399.
How much is Zepbound on TrumpRx?
TrumpRx shows the Zepbound 2.5 mg vial at $299 and sends the order to LillyDirect. There, the vial is $399 at 5 mg and $449 at 7.5 mg to 15 mg if you refill within 45 days; otherwise the regular price is $499 at 7.5 mg and $699 at 10 mg and above.
Can I use TrumpRx if I have Medicare?
Not through your Medicare benefit. The coupons are for self-pay patients and can't be combined with Medicare, Medicaid, the VA, TRICARE or private insurance. If you have Part D, check whether you qualify for Medicare GLP-1 Bridge, which covers Wegovy, Foundayo and the Zepbound KwikPen for $50 a month.
